What is family-managed home care?

Family-Managed Home Care is a new model for home care delivery in Ontario. It was designed to provide more control over care to eligible clients and families. Through the program, eligible clients receive funding that they can use to purchase home care services or employ care providers.

The program is unique because it allows care clients and their families more flexibility since they can choose a provider and schedule that best fits their needs, instead of being assigned a care provider.

Who is eligible for family-managed home care?

Clients must first go through an assessment by the LHIN (Local Health Integration Network) to determine their needs and meet the eligibility requirements for traditional home care. Each client is assessed by the LHIN on an ongoing basis to determine the support that will be funded. You can then engage a provider like Meaningful Living to provide the services requested.
